After learning and making your decisions, it's time to enroll:
Generally, any 2021 elections you make
will remain in effect through Dec. 31, 2021. When your enrollment period ends, you may only make a change to your
benefit elections during the year if you experience a qualified life event and report it through MyHR within 30 calendar days of life event.
The elections you make or default coverage you receive will remain in effect through the end of the year and you will only be permitted to make changes to these elections during the plan year if you experience a qualified life event and report it through MyHR within 30 calendar days of the life event effective date.
Note: Health Savings Account contributions can be changed at any time on MyHR.
ID Cards
If you enroll in the health plan, you and your covered family members will receive one combined Anthem Blue Cross and Blue Shield (medical/vision) and CVS Caremark (prescription) card in the mail within 30 days of your enrollment. You also should receive a Health Savings Account (HSA) debit card from Anthem if you enroll in either the Green or Orange plan.
If you need to visit your doctor or use your pharmacy benefits before you receive your new ID cards, you can print temporary ID cards.
To print your Anthem Blue Cross and Blue Shield temporary ID card,first call 1-855-285-4212 to request your ID number. Then, using your ID number,
visit Anthem.com to sign up and follow the steps to print off a
temporary ID card.
If you enroll in the dental plan, you will not receive a card in the mail and don’t need one. You can print one on the MetLife website.
