The information provided in the Open Enrollment Guide and in the additional notices below constitutes a Summary of Material Modification to the Assurant Health and Welfare Plan and the various plans comprising the Assurant group benefits program (Plans). This information should be retained as an update to the latest Assurant Health and Welfare Benefit Plan Summary Plan Description (SPD).

While the company intends to continue these benefits, it reserves the right to change or terminate them at its sole discretion. The terms of the Plans cannot be modified or amended by oral statements.

Women's Health and Cancer Rights Act of 1998 Notice

The Women's Health and Cancer Rights Act of 1998 mandates that if you are covered under a group or individual health plan that provides medical and surgical benefits with respect to a mastectomy, you also have coverage available for the following procedures and care:

 

Reconstruction of the breast on which the mastectomy has been performed;

 

Surgery and reconstruction of the other breast to produce a symmetrical appearance;
and

 

Prostheses and treatment of physical complications at all stages of mastectomy, including lymphedema.

 

Coverage will be subject to your health plan's payment provisions.

Newborns' and Mothers' Health Protection ActGroup health plans and health insurance issuers generally may not, under federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother of a newborn child to less than 48 hours following a normal vaginal delivery, or less than 96 hours following a cesarean section. However, federal law generally does not prohibit the mother's or newborn's attending physician, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, health plan providers may not require that a provider obtain authorization for prescribing a hospital length of stay of less than 48 hours (or 96 hours).

Transparency in Coverage

 


 

The Transparency in Coverage Final Rules require certain group health plans to disclose information on a public website regarding in-network provider rates and historical out-of-network allowed amounts and billed charges for covered items and services in two separate machine-readable files (MRFs). The MRFs for the benefit package options under the Assurant Health & Welfare Benefit Plan can be found as follows:

Please note that to correctly access these files, you will need our Employer Identification Number (EIN) which is 39-1126612. The EIN must be entered in this exact format with a dash after the first two digits to properly conduct the MRF search.
