How to Find a Provider in the Network

The Anthem network for Assurant is based on where you live and the health plan you're enrolled in, not where your doctor or facility is located. You may use the links and instructions below to search for a provider in the network.

If you are enrolled in the Assurant health plan, you may also visit the Anthem Blue Cross and Blue Shield member website, Anthem.com, and if you’re in the Blue, Green or Orange plan, you may also use the Sydney Health mobile app to search for providers in your area.

Click on your state of residency from the list provided on the Anthem Network Provider Search website. Once open, enter your zip code in the search bar at the top of the screen. A link to your Blue, Green and Orange health plan (in-network only) provider list will appear in the 4th column. Click the link to access the Anthem’s Find Care tool and search for doctors and providers in your plan’s network. On Anthem’s Find Care page, enter your zip code again, and then search for specific doctors, or you can scroll down and search for specific types of providers (eg: Primary Care, or Hospitals).	Click on your state of residency from the list provided on the Purple Plan Network Provider Search website. Once open, enter your zip code in the search bar at the top of the screen. A link to your Purple health plan (in-network only) provider list will appear in the 4th column. Click the link to access the Anthem’s Find Care tool and search for doctors and providers in your plan’s network. On Anthem’s Find Care page, enter your zip code again, and then search for specific doctors, or you can scroll down and search for specific types of providers (eg: Primary Care, or Hospitals).
